Make an appointment or let us know something changed?

  • You can contact ACTA by phone to make or change appointments.
  • Inform ACTA when your address or health insurance arrangements changes. This is to avoid errors.

When registering, each patient will receive an ACTA patient card, bearing contact information. Details of future appointments will be entered on the inside of your card. A sticker bearing your personal patient number is attached to the back of this card. Keep details of this number handy when contacting ACTA to help us provide you with the best possible assistance.
